  The Prescription Information Sheet is provided to you for submitting to your insurance company for filing claims.
   
 

INVOICES

An invoice is included with your order.  Upon receipt of an order, payments can be made in several

 

convenient ways:

   
 
  • Call an account representative at 1-800-742-0516 (press Option 3) to pay by credit card.  At your request, we can keep your credit card on file for future payment(s).

               

     
 
  • Mail your payment (credit card information or check) with the invoice stub in the envelope that is provided with your order.

 
  • On our secured website, payments can be made online.

     
 

STATEMENTS

Statements are sent monthly to provide detailed information on your account, including the balance owed. 

 

The closing date of your statement is at the end of each month.  Payments received AFTER the closing

 

date will appear on your next month's statement.
