How do I pay for the
medication?
We accept checks, money orders, and major credit cards (Visa, MasterCard, American Express, and Discover). Credit card payments are accepted over
the phone and online. If you prefer to mail your payment with the envelope provided, make checks payable to Pet Health Pharmacy. All invoices must be paid in full before your

How do I get a prescription through
you?
- Call. The quickest way to obtain a prescription is to have your pet's veterinarian call in the prescription
at 1-800-742-0516, or the veterinarian's office may fax the prescription to 1-866-373-0030.
- Mail. If your pet's veterinarian has given you a written prescription, you may mail it to us at Pet Health
Pharmacy, 12012 N. 111th Ave., Youngtown, AZ 85363.
- Fax. You may fax the prescription to us at 1-866-373-0030 (due to state regulations, pharmacies are required
to call the veterinarian for verification of the prescription, so faxing may cause a slight delay in processing the medication).
How is the prescription shipped? Is there a shipping
charge?
Prescriptions (excluding medications that require refrigeration) are shipped via Priority Mail® (United States Postal Service) at the cost
of $5.35 which includes Delivery Confirmation™ capabilities. If your prescription does require refrigeration, it is generally shipped FedEx® Overnight at the current
charge of $21.00, Monday - Thursday. Friday shipment for Saturday delivery is extra, if available in your area. Other expedited delivery services such as FedEx®, UPS®, and
USPS Express Mail® are also available upon request.

Do you accept my insurance
plan?
We do not accept insurance; however, we can provide you with the necessary documentation for submission to your insurance company.
Do you carry Heartguard® and Frontline®
Plus?
Pet Health Pharmacy specializes in compounding medications; therefore, we do not carry these manufactured products. Contact your veterinarian for
these prescriptions.
How much time should I allow for my prescription refill to
arrive?
You should allow 5-7 business days to receive your refill after placing your order with our pharmacy (our number is 1-800-742-0516).
If the bottle says there are no refills remaining, do I
contact my veterinarian or does Pet Health Pharmacy?
To expedite a renewal of refills, you may call your veterinarian or we will be happy to contact your veterinarian for you! Once approved by your
veterinarian, please allow 5-7 business days to receive your prescription.
Why do I receive a statement when I have already paid the
invoice enclosed with my prescription?
Pet Health Pharmacy's billing cycle closes at the end of the month; therefore, your payment may not have been received or credited to your
account prior to processing and mailing of statements.
